The Hardship: Managing "Crisis" Without the Cash

In early 2026, the Ligas Court Monitor officially found the Illinois Department of Human Services (IDHS) "not in substantial compliance" for the twelfth year. The state is under court order to accelerate transitions from institutional settings to community-based settings (CILA), but the funding for these "Crisis Transitions" is frequently delayed.

RCM Insight: The "Specialized Tier" Billing Lag

Providers are being asked to take high-acuity Class Members with promised "Specialized Tier" rates. However, the PUNS (Prioritization of Urgency of Need for Services) database often lags behind actual service delivery. RCM teams are finding that while they have a person in a bed, they are stuck billing the "Base" rate for months while waiting for the Division of Developmental Disabilities (DDD) to approve the specialized funding tier.
